If you would like to apply for help on the SWH&H Workday, please fill out the application here. The application deadline for the May 2024 Workday is January 30, 2024.

How to Apply for Help

You can fill out the Apply for Help application for yourself or call or email us.

As the homeowner, please print and sign this Homeowner Waiver/Release form and submit it to the SWH&H team.

Once your application is received, you can expect a follow up phone call from us within 1-2 weeks.

If you have an emergency and need to contact the Home Emergency Action Repair Team (H.E.A.R.T.) or have not received a follow-up call from us on a project submission, please contact us via email or phone at (360) 221-6063.
